§For QME, AME and IME Physicians

The record, organized. The opinion, yours.

A 1,200-page record set arrives. PhyPal classifies every document, builds a chronology with each entry linked to its page, extracts the elements your cover letter asks about and flags what is missing or contradictory — before you read the first page. You review, you form the opinion, you write and sign the report.

The deposition test

Can you show what was reviewed, what was relied on, and where each fact came from?

That is the question an attorney asks, and the question California Labor Code §4628 is built around. A summary that cannot be checked is a liability. An evidence layer where every material fact carries its page is a defense. PhyPal is built to the second standard.

  • Every chronology entry and extracted element links to its source page
  • Gaps — missing date ranges, referenced-but-absent imaging, conflicting histories — are surfaced, not smoothed over
  • Report structures are defined by you, for the evaluation being performed
  • PhyPal prepares evidence; it does not write the report or form the opinion

Labor Code §4628, in one paragraph

No one other than the signing physician may take part in the non-clerical preparation of a medical-legal report. If anyone else prepares an initial history or excerpts records, the physician must review that work and make further inquiry as needed, and the report must name everyone who participated. Non-compliance makes the report inadmissible and forfeits the fee.

What you receive

Evidence extraction for QME, AME and IME evaluations.

Ingest→Classify→Extract→Verify→Structure→Physician review

Medical chronology

Document and encounter classification; a dated chronology built from the record, not from memory, with every entry linked to its page.

Issue-specific extraction

Mechanism, prior history, diagnostics, treatment and response, work status — organized to the questions the cover letter poses.

Source provenance

Fact-to-page traceability for every material fact, so verification takes seconds and deposition prep starts from the evidence.

Gap and inconsistency flags

Missing records, contradictory histories, imaging referenced but not present — surfaced explicitly.

Physician-defined structures

Your evaluation structure, your sections, your order of review — configured once, reused for every case.

QA and feedback

Completeness checks, your corrections captured and the workflow refined over time.

FAQ

What evaluators ask.

Is this medical record review?

No. The review is yours. PhyPal is evidence infrastructure for clinical records: it ingests, classifies, extracts and structures the submitted records and links every material fact to its source so your review is complete, faster and defensible.

Does PhyPal write any part of the report?

No. PhyPal prepares the evidentiary foundation — chronology, extracted elements, source links, gap flags. You review the records, form the opinion, and write and sign the report. How PhyPal's role is disclosed under §4628 is something we work through with each evaluator.

How are the records handled?

Under HIPAA-aligned safeguards, with access controls and audit trails. Records are processed for your evaluation and your evaluation only.

How long does preparation take?

Turnaround depends on record volume and is defined per engagement. Turnaround time, source-link coverage and omissions found in QA are among the metrics every evidence pilot reports.
